Dear Patient,

We are honored that you have chosen to allow us to provide dental services for you and look forward to your visit.   Please take a few moments to read this letter and comply with our request.  We believe that this will allow your visit to go smoother and more satisfactory for you as well as our staff.

Enclosed are 3 forms we would like for you to complete 1) the registration form 2) the health history form 3) the insurance verification form (if applicable).  Please fill these forms out COMPLETELY.  Regarding the registration form, please include an emergency contact not living with you. This allows us to contact you should your phone line be out of order. The health history form is for Dr. Baptiste to read so she can learn about issues that may be important to your dental care.  The insurance verification form is very important and must be filled out before you are seen. Please call your insurance to verify benefits and make sure you get the name of the person with whom you speak. If you do not have insurance please disregard this form.   We thank you for taking the time to complete these forms BEFORE your scheduled appointment; not doing so could result in a longer wait time in the reception area. In addition to bringing in the completed forms please bring, your insurance card as well as a photo ID with you when you come for your appointment. These forms may also be faxed back to us but please bring the originals with you.  Your appointment will be rescheduled if you do not bring in or fax front and back of your insurance card.

We look forward to seeing you. If you are unable to keep your appointment please give us the courtesy of a call at least 24 hours before so that we may use your time for someone else requiring our service. Please note there is a charge for ALL broken appointments.
