Policy
for
Cheryle Baptiste D.D.S.,
P.C.
Our Primary goal is to provide you, your
family and friends the best dental care possible in a pleasant and
caring environment. We
want to ensure a positive experience for you while visiting our
office. At the same
time we want to avoid any un-necessary confusion, hence, we will
present our office financial policy to you.
We ask that you make payment in full
at the time of time of your visit. You should be aware of and be
prepared to pay all fees charged for services prior to treatment.
We ask this of ALL our
patients, old and new alike. Following your comprehensive oral
and radiographic examination we will submit your treatment plan to
your insurance company for preauthorization. Once we receive the written
preauthorization from your insurance company then we will accept
your co-payment and submit to your insurance company for their
portion. The ONLY
exceptions are for those who have made prior arrangements with our
financial coordinator.
If you choose not to wait for your insurance company
preauthorization then as a courtesy to you, we will submit your
insurance forms. This
however, will only be done after payment has been remitted for
services rendered. Please
keep in mind that you are responsible for your bill, not your
insurance company!
There is a $30 FEE for ALL Broken Appointments
and Late Cancellations (less than 24 hours): NO
EXCEPTIONS!
Methods of
Payment
We accept Care Credit (A
dental line credit the 1st 90 days are the same as cash), Capital One Health Care
Finance, MasterCard, Visa, Travelers Checks, Money
Orders, Personal Checks and Cash. For your convenience, we also
accept your Personal Bank Check Cards. There are no refunds for the
Capital One Dental Fee Plan and the Care Credit Plan.
